Motor No. KYA00E041786; File No. 0620-198464, Chassis No. KYA00E00E041786 and MVRR No. 1947046010. That the owner’s copy of Certificate of Registration of the said vehicle was lost and despite of diligent efforts exerted by the owner to locate the said CR, it could no longer be found and considered as loss, executed this AFFIDAVIT OF LOSS CR/WARRANTY on July 5, 2021 and entered as per Doc. 503, Page No. 102, Book No. IV, Series of 2021 of notarial registry of Atty. Geline Joy D. Samillano, a notary public for and in the City of Roxas, Province Capiz.
