In compliance with Section 5 of R.A. No. 9048, a notice is hereby served to the public that AMALIA D. AGNAS, has filed with this Office a petition for Change of First Name from “LYDIA” to “AMALIA”, in the birth certificate of LYDIA DIAZ DELIMA who was born on January 25, 1964 at Sapian, Capiz and whose parents are FELIPE DE LA CRUZ DELIMA and SANCHA ORE DIAZ.
Any person adversely affected by said petition may file his/her written opposition with this Office not later than fourteen (14) days from the date of this notice.
