In compliance with Section 5 of R.A. No. 9048 notice is hereby served to the public that MA. ANA A. SILVESTRE has filed with this office a petition for change of Child’s First Name from “MARIA ANA” to “MA. ANA”, correction of Child’s Date of Birth to read as “MARCH 24, 1964” and Child’s Place of Birth from ROXAS CITY, DUMOLOG CAPIZ, CAPIZ” to “DUMOLOG, ROXAS CITY, CAPIZ” in the Birth Certificate of MARIA ANA ALOAD APELADO born on MARCH TUESDAY 24, 1964 at Roxas City, Dumolog Capiz, Capiz and whose parents are Cezar Amarillo Apelado and Enriguita Alocada Aload.
Any person adversely affected by said petition may file his/her written opposition with this office not later than (14) days from date of this notice.
