In compliance with the publication requirement and pursuant to OCRG Memorandum Circular No. 2013-1. Guidelines in the Implementation of the Administrative Order No. 1 Series of 2012 (IRR on R.A. 10172) notice is hereby served to the public that JONATHAN O. GALLARDO has filed with this office a petition for correction of Child’s First Name from “DIONATAN” to “JONATHAN” and Child’s Date of Birth from “AUG. 12, 1961” to “AUGUST 13, 1961” under R.A. 10172 in the Birth Certificate of DIONATAN OQUENDO GALLARDO who was born Aug. 12, 1961 at Roxas Avenue, Roxas City, Capiz, and whose parents are Alberto Ortiz Gallardo and Libertad Obidos Oquendo.
Any person adversely affected by said petition may file his/her written opposition with this office not later than (14) days from date of this notice.
