In compliance with section 5 of R.A. No. 9048 notice is hereby served to the public that MARYLON D. BILLANES has fi led with this office a petition for change of Child’s First Name from “MARINA” to “MARINA LEVENE” and Correction of Mother’s Middle Name from “BALIDO”
to “BOLIDO” in the Birth Certifi cate of MARINA CELINO DALIVA born on March 3, 1960 at St. Anthony Hosp., Roxas City, Capiz and whose parents are Faustino Capapas Daliva and Felicisima Balido Celino.
Any person adversely affected by said petition may file his/her written opposition with this offi ce not later than (14) days from the date of this notice.
