the owner of (1) one unit of ISUZU
vehicle with Plate No.: WDZ204;
Chassis No. NKR58E-7150409;
Motor No. 4BE1-992543 and File
No. 1368-129994. That the original
copy of Certificate of Registration of
the said vehicle was lost and despite
of diligent efforts exerted by the
owner to locate the said CR, it could
no longer be found and considered as
loss, executed this AFFIDAVIT LOSS
CERTIFICATE OF REGISTRATION
on December 28, 2023 and entered as
per Doc. 102, Page No. 21, Book No.
26, Series of 2023 of notarial registry
of Atty. Luzermindo A. Calmorin.,
a notary public for in the City and
Province of Iloilo.
