21, 2023, has left a Savings Deposit Account with the Philippine Veterans Bank – Roxas City Branch under
Savings Account No. 0049-052524- 101 with a balance of Php333,053.10. That the HEIRS hereby request the Philippine Veterans Bank – Roxas City Branch to release the balance of the said deposit account in favor of FELICIDAD D. SILVESTRE and/ or to transfer it to an account in her name. Subject of Affi davit of Claim with Waiver of Rights before notarial registry of Atty. Nichelle Hope A.
