Notice is hereby given that TOMAS R. ARCENAS was the owner of one (1) unit of HONDA motor vehicle with Plate No. 5298-FV; Chassis No. XRM14124737; Motor No. XRM14E124738 and File No. 0636-00063746712. That the owner’s copy of Certificate of Registration of the said vehicle was lost and despite of diligent efforts exerted by the owner to locate the said CR, it could no longer be found and considered as loss, executed this AFFIDAVIT OF LOSS CERTIFICATE OF REGISTRATION on March 01, 2021 and entered as per Doc. 91, Page No. 20, Book No. XXXIV, Series of 2021 of notarial registry of Atty. Sheila Arenajo-Artillero, a notary public for and in the City of Roxas, Province of Capiz.
