Notice is hereby given that GLEN BARCENA was the owner of one (1) unit of REBUILT motor vehicle with Plate No. VBN486; Motor No.3K73538; Chassis No. ALB2001-0496C and MV File No. 0412-00000117867. That the owner’s
copy of Certificate of Registration of the said vehicle was lost and despite of diligent efforts exerted by the owner to
locate the said CR., it could no longer be found and considered as loss, executed this AFFIDAVIT OF LOSS CERTIFICATE OF REGISTRATION WITH UNDERTAKING on August 25, 2020 and entered as per Doc. 243, Page No. 52, Book No. CXXV, Series of 2020 of notarial registry of Atty. Benjamin B. Distura, a notary public for and in the City of Roxas, Province of Capiz.
