Notice is hereby given that CARLITO JR POLICARPIO ARMADA was the owner of one (1) unit of Isuzu/Jeep vehicle with Plate 063601; Motor No. 486518; MV File No. 0472-66865; Chassis No. SPMM6262493C and CR No. 20547755. That the owner’s copy of Certificate of Registration of the said vehicle was lost and despite of diligent efforts exerted by the owner to locate the said OR and CR, it could no longer found and considered as loss, executed this AFFIDAVIT OF LOSS CR WITH UNDERTAKING on August 05, 2021 and entered as per Doc. 119, Page No. 25, Book No. CXXVII, Series of 2021 of notarial registry of Atty. Benjamin B. Distura, a notary public for and in the City of Roxas, Province Capiz.
