Notice is hereby given that GODOFREDO A. BOLVES JR. was the owner of one (1) unit of KAWASAKI MOTORCYCLE with Plate No. 063610; Chassis No.
BC175GB02648 and Engine No. BC175AEAP1711. That the owner’s copy of Certificate of Registration of the said vehicle was lost and despite of diligent efforts exerted by the owner to locate the said CR, it could no longer be found and considered as loss, executed this AFFIDAVIT OF LOSS CR WITH UNDERTAKING on September 29, 2020 and entered as per Doc. 36, Page No. 09, Book No. CXXVI, Series of 2020 of notarial registry of Atty. Benjamin B. Distura, a notary public for and in the City of Roxas, Province of Capiz.
