Notice is hereby given that MARLO J. BAYHON was the owner of one (1) unit KAWASAKI motor vehicle with Motor No.
BC175AEAE4897; File No. 063600063769558; Chassis No. BC175ABA7206; Plate No. 3427FU and CR No. 17214699. That the owner’s copy of Certificate of Registration of the said vehicle was lost and despite of diligent efforts exerted by the owner to locate the said CR, it could no longer be found and considered as loss, executed this AFFIDAVIT OF LOSS CR WITH UNDERTAKING on July 22, 2021 and entered as per Doc. 454, Page No. 94, Book No. CXXVII, Series of 2021 of notarial registry of Atty. Benjamin B. Distura, a notary public for and in the City of Roxas, Province of Capiz.
