(1) unit 2003 NISSAN PICKUP Motor Vehicle with Motor No.: TD27-TE04334; Serial No.: CVPGLFFD22-C83676; Plate No.: FEW-636 and MV File no.: 0604-43325. That the CR issued by the Land Transportation Offi ce (LTO) was kept inside the enveloped together with other important documents are missing or maybe someone stole it AFFIDAVIT OF LOSS (LTO CERTIFICATE OF REGISTRATION) or slipped during travel or typhoon and despite of diligent eff orts exerted by the owner to locate the said CR, it could no longer be found and considered as loss.
