Notice is hereby given that SHERYL D. ROXAS / ERWIN H. ROXAS / PEMDI was the registered owner of (1) one unit motor vehicle described as; MAKE & TYPE: YAMAHA; MOTOR NO.: G3J1E-0558933; CHASSIS NO.: MH3SG4650L0096300; PLATE NO.: 947 FUN and FILE NO.: 0601-00000911749. That the original copy of the said Certificate of Registration issued by the LTO was lost despite of diligent searched exerted by the owner to locate the said C.R. it could no longer be found and considered as loss. They warrant no liability whether civil or criminal will be imputed against the Land Transportation Office subject to Affidavit of Loss of Certificate of Registration with Undertaking. Likewise, they executing to attest the truth of the foregoing facts and to request the LTO to issue a duplicate copy in lieu of the lost one. Entered as per Doc.
