ENCISO was the owner of two (2) units of TRAILER HEAVY motor vehicle with Plate No. TUC-834 & TUC-512; Serial No. ASHD201110501C & ASHD201110480C; MV File No. 1358-00000142893 & 1304-00000271105; CR No. 11263801-3 & 12172700-1 and OR No. 711568966 & 1996017391. That the owner’s copy of Certificate of Registration and Official Receipt of the said vehicle was lost and despite of diligent efforts exerted by the owner to locate the said LTO CR and OR, it could no longer be found and considered as loss, executed
this AFFIDAVIT OF LOSS (LTO CERTIFICATE OF REGISTRATION & OFFICIAL RECEIPT) on April 19, 2021 and entered as per Doc. 6, Page No. 3, Book No. CXIV, Series of 2021 of notarial registry of Atty. Judgee Earl Ross D. Del Castillo, a notary public for and in the City of Roxas, Province of Capiz.
