Notice is hereby given that SUSAN G. GUZMAN was the registered owner of (1) one unit motor vehicle described as; MAKE: HONDA; BODY TYPE: MOTORCYCLE; CHASSIS NO.: KCN02035G01525E; ENGINE NO.: KCN02035001525 and PLATE NO.: FL4254. That the original copy of the said Certificate of Registration issued by the LTO was lost despite of diligent effort exerted by the owner to search the said C.R. it could no longer be found and considered as loss. She executed an affidavit to attest the truth of the stated facts, and to request the LTO to issue a new copy of Certificate of Registration in her favor in lieu of the lost one. Entered as per Doc.
