Notice is hereby given that MA. ELIZA A TRINIDAD, was the holder of OR and CR of Ramfire
TC 150 black and registered to Edna Isiderio Abalayan with Engine Nos. RF162FJKMK3A023266K
and Chassis No. RFCPCKXJ2JBO23266K. That during the typhoon Ursula on December 24, 2019, their house was devastated by the said typhoon and their belonging were was out included the CR and OR of the above- mentioned vehicle and reasonable search and inquiry has been exerted by the undersigned to locate the same, but all efforts prove futile, to no avail. That she executing this affidavit to attest the foregoing facts regarding the lost of the above-mentioned OR and CR and she further request to the office concerned to consider this affidavit and for whatever legal purpose and intent this affi davit may
