Arcenas, was the registered owner of one (1) unit HYUNDAI SANTA FE 2.2L sports utility vehicle with Chassis No. KMHSUS1XBEUSR1043; Plate No. AEA2327; Engine No. D4HBEU061090 and MV File No 130100000244648. That the owner’s copy of Certificate of Registration of
the said motor vehicle was lost and despite of diligent effort exerted to locate the said C.R., it could no longer found and considered as lost. That she is executing this affidavit to attest to the truth of the above-stated facts and for whatever purpose it may serve. Entered under notarial registry of Atty. Genalyn B.
