Notice is hereby given that the intestate estate of the late SPOUSES JOSELITO J. ORTENCIO married
to MELINDA A. ORTENCIO has left a savings account from the following banks: A. RURAL BANK OF MAMBUSAO INC. with account no.: 3085-001-01-009868-0; B. METRO BANK ROXAS BRANCH
with account no.: 141-3-14133416; C. PHILIPPINE NATIONAL BANK with account no.: 311710062450.
That the pursuant to the Rule 74, Sec. 1 of the Rules of Court, that the sole heir hereby adjudicate unto himself the aforesaid bank accounts and the money deposited in it name as well as any and all liabilities arising from the said bank accounts, which represents his lawful inheritance, and hereby files the same with the concerned government agency/ instrumentality with the request that the said adjudication be made effective without judicial proceedings as prescribed.
