Petitioner request authority for the Change Party Applicant in the Certificate of Public Convenience to operate a PUJ service on the line: ROXAS CITY – CAMP PERALTA (JAMINDAN) and vice versa the use of
ONE (1) unit/s. from GALO L. VELADO – 1st party (deceased) in favor of ALLEN L. VELADO – 2nd party (heir).
This application/petition shall be considered by the Board on OCTOBER 25, 2022 at 9:00 A.M. thru ONLINE HEARING. At least FIVE (5) days prior to the date of hearing, applicant/petitioner shall publish this NOTICE once in a newspaper of general circulation in Region VI.
Proof of the publication (Newspaper with Affidavit of Publisher) and Officer of Exhibits must be submitted at least three (3) days before the scheduled online hearing.
Together with the foregoing proof of publication, applicant and counsel are directed to submit their email addresses.
Parties opposed to the granting of this application/petition must file their written opposition supported by the documentary evidence unless the Board deems it necessary to receive additional documents and/or oral evidence.

PETITION
COMES NOW, Petitioner and unto this Honorable Board, most respectfully alleges;
That Petitioner-First Party is of legal age, single/married, Filipino citizen and residing at Jamindan, Capiz, PHILIPPINES.That Petitioner-First Party died intestate without leaving any debts of obligations of whatever kind of nature last JUNE 29, 2021 at Jamindan, Capiz. Attached as “Annex A” is the machine copy of CERTIFICATE OF DEATH issued by OFFICE OF CIVIL REGISTRAR to prove the death of the FIRST PARTY;
That Petitioner-Second Party is son and surviving heir of GALO L. VELADO, of legal age, Filipino citizen and residing at Jeana Norte, Jamindan, Capiz, Attached as Annex “B” is the copy of DEED OF ADJUDICATION WITH WAIVER OF RIGHTS, Annex “C” is the machine copy of TIN card issued by BIR to prove the citizenship and legal identity of Petitioner-Second Party.
That Petitioner-First Party is a grantee of a Certifi cate of Public Convenience to operate a PUJ service along the line: ROXAS CITY – CAMP PERALTA (JAMINDAN) and vice versa with the use of ONE (1)
unit/s issued a Provisional Authority (PA) on April 01, 2022, and said unit described as follows;

That the CPC will expire on MARDH 31, 2023. Attached is OR and CR as Annex “D and D-1” and copy of Franchise Confirmation as Annex “E”.
That Petitioner-Second Party wishes to apply for change party applicant/transfer of said Certificate of Public Convenience (CPC) including unit described above;
That there is still an urgent necessity to on the line and route applied herein;
That petitioner is financially capable to operate and maintain said unit and willing to comply with all rules and regulations of the Honorable Board may impose from time to time.

WHERE, IT IS MOST RESPECTFULLY PRAYED UNTO THIS Honorable Board that this petition be granted. Other remedies just and equitable to the premises are hereby prayed for.
