VALENCIA has filed with this office a petition for change of Child’s First Name from “ANGELES SASHKIA” to “SASHKIA” in the Birth Certificate of ANGELES SASHKIA VALENCIA ANGELES born on September 27, 1994 at Baybay, Roxas City, Capiz and whose parents are Pedro Ocampo Angeles and Rowena Albason Valencia.
Any person adversely affected by said petition may file his/her written opposition with this office not later than (14) days from date of this notice.
