In compliance with section 5 of R.A. Act 9048 notice is hereby served to the public that JOSE IV D. DIAZ has filed with this offi ce a petition for change of Child’s first name from “FLORENCE” to “MA. FLORENCE”
and correction of Mother’s First Name from “FLORENCIA” to “FLORENCITA” in the Birth Certifi cate of FLORENCE DIAZ born on April 13, 1966 at Capiz Emmanuel Hospital, Roxas City Capiz and whose parents are Jose Diaz, III and Florencia Dorado.
Any person adversely aff ected by said petition may fi le his/her written opposition with this office not later than (14) days from the date of this notice.
