BEBOSO has filed with this office a petition for change of Child’s First Name from “MARIA LOURDES” to “MA. LOURDES” correction of Child’s, Father’s and Informant’s Last Name from “FONTINALGO” to “FANTINALGO” in the Birth Certificate of MARIA LOURDES BARTOLO FONTINALGO born on Jan. 27, 1970 at St. Anthony Hospital, Roxas City, Capiz and whose parents are Mamerto Freirez Fontinalgo and Milagros Bartolo.
Any person adversely affected by said petition may file his/her written opposition with this office not later than (14) days from date of this notice.
