In compliance with section 5 of R.A. Act 9048 notice is hereby served to the public that MELODY B. LLARVEZ has filed with this office a petition for change of Child’s First name from “MA. MILODY” to “MELODY” and correction of Father’s Last Name from “BARES” to “BARRES” in the Birth Certificate of MA. MILODY BENTILLO BARES born on September 2, 1957 at Loctogan, Roxas City, Capiz and whose parents are SANTIAGO SILOS BARES and PACITA CAPOTE BENTILLO.
Any person adversely affected by said petition may file his/her written opposition with this office not later than (14) days from the date of this notice.
