In compliance with Section 5 R.A. Act 9048, a notice is hereby served to the public that SHEILA JANE E. CLASIN has filed with this Office a petition for change of child’s first name from “HEDILYN” to “SHEILA JANE” in the birth certificate of HEDILYN MAYUGA ASCOBRO who was born on JANUARY 2, 1981 at DAO, CAPIZ, PHILIPPINES and whose parents are WENCESLAO ESCUBRO and LEONORA MAYUGA.
Any person adversely affected by said petition may file his/her written opposition with this office not later than (14) days from the date of this notice.
