In compliance with section 5 R.A. Act 9048, a notice is hereby served to the public that ABADISA A. MACAPALLAG has filed with this office a petition for change of First Name of her daughter from “ROXAS” to “ROXAN” in the birth certificate of ROXAS ACUYONG MACAPALLAG, who was born on March 10, 1983 at Brgy. Agustin Navarra, Ivisan, Capiz and whose parents are JOHNNY GUMANGAN MACAPALLAG and ABADESA VALDESIMO ACUYONG.
Any person adversely affected by the said petition with this office may file his/her written petition with this office not later than fourteen (14) days after the last publication of this notice.
