In compliance with the publication requirement and pursuant to OCRG
Memorandum Circular No. 2013-1 Guidelines in the Implementation of
the Administrative Order No. 1 Series of 2012 (IRR on R.A. 10172), a
notice is hereby served to the public that MELO P. DERRAMAS, has filed
with this Office a petition for Change of First Name from “CARMELO”
to “MELO” and Correction of entry in his date of Birth from “October 16,
1965” to “April 20, 1965” in the certificate of live birth of CARMELO P.
DERRAMAS at Maayon, Capiz and whose parents are BUENAVENTURA
DERRAMAS and CONSUELO PAROM.
Any person adversely affected by said petition may file his written
opposition with this office not later than 14 days from the date of this
notice.
