NOTICE FOR PUBLICATION
In compliance with Section 5 of R.A. Act No. 9048, a notice is hereby served to the public that TEDDY B. DELEŇA, has filed with this Office a petition for change of first name from “JOSE TEDDY” to “TEDDY” in the birth Certificate of JOSE TEDDY B. DELEŇA who was born on June 16, 1955 at Maayon, Capiz and whose parents are Agaton Deleña and Florenda Besa.
Any person adversely affected by said petition may file his/her written opposition with this office not later than fourteen (14) days from the date of this notice.
