In compliance with section 5 of R.A. Act 9048, a notice is hereby served to the public that GENALYN RIVAS LABAO has filed with this office a petition for change of first name from “MA. JANELYN (OCRG COPY)”/“MA. JAMELYN (REGISTER OF BIRTHS)” to “GENALYN”
in the Birth Certifi cate of MA. JANELYN LINDOG RIVAS who was born January 14, 1971 at Mambusao, Capiz and whose parents are Bernardo Villanueva Rivas and Erlinda Lusano Lindog.
Any person adversely affected by said petition may file his/her written opposition with this Office not later than fourteen (14) days from
the date of this notice.
