NOTICE FOR PUBLICATION
In compliance with section 5 R.A. Act 9048, a notice is hereby served to the public that WENDEL LACUARTA CELESTIAL has filed with this Office a petition for change of first name from “WENDELINO” to “WENDEL” in the birth certificate of WEDELINO LACUARTA CELESTIAL who was born December 23, 1961 at Mambusao, Capiz and whose parents are NAPOLEON LAVEREZ CELESTIAL and CAROLINA ANDALECIO LACUARTA.
Any person adversely affected by said petition may file his/her written opposition with this Office not later than fourteen (14) days from the date of this notice.
