In compliance with section 5 R.A. Act 9048, a notice is hereby served to the public that LEONARD B. BAGARES has filed with this office a petition for change of first name from “ANTONIO LEONARD” to “LEONARD” in the birth certificate of ANTONIO LEONARD BILLONES BAGARES who was born on December 20, 1961 at Panay, Capiz and whose parents are NICOMEDES B. BAGARES and NENA B. BILLONES.
Any person adversely affected by said petition may file his/her written opposition with this Office not later than fourteen (14) days after the last publication.
