BUHAT has filed with this office a petition for Change of First Name from MOMIE to MOMITA in the Certificate of Live Birth of MOMIE A. BUHAT who was born on June 27, 1971 at TANZA NORTE, PANAY, CAPIZ and whose parents are MAURO B. BUHAT and BERNARDITA S. ALIANZA.
Any person adversely affected by said petition may file his/her written opposition with this Office not later than January 24, 2026.
