In compliance with Section 5 of R.A. No. 9048, a notice is hereby served to the public that LEONIL FUENTES DELAY, has filed with this Office, a petition for Change of First Name of his Sister from
“JOSEFINA” to “JOSEPHINE.” in the Birth Certifi cate of JOSEFINA FUENTES DELAY who was born on July 8, 1961 at Panitan, Capiz and whose parents are Arsenio Delay and Erlinda Fuentes.
Any person adversely affected by said petition may file his/her written opposition with this Office not later than fourteen (14) days from the date of this notice.
