In compliance with the publication requirement and pursuant to OCRG Memorandum Circular No. 2013-1. Guidelines in the Implementation of the Administrative Order No. 1 Series of 2012 (IRR on R.A. 10172). Notice is hereby served to the public LYNEE DISTOR TAMAYO, has filed with this Office, a petition for change of name from “MA. LYNE” to “LYNEE” and correction on her Date of Birth from JULY 3, 1957 to MAY 1, 1957 in the certificate of live birth of MA. LYNE D. DISTOR who was born on July 3, 1957 at Panitan, Capiz, and whose parents are Santos Distor and Epeginia Delfin.
Any person adversely affected by said petition may file his/her written opposition with this Office not later than fourteen (14) days from the date of this notice.
