In compliance with section 5 of R.A. No. 9048, a notice is hereby served to the public that ARNOLD BORLEO APONESTO, has filed with this Office a petition for change of first name from “JOSE ARNOLD” to “ARNOLD” in the birth certificate of JOSE ARNOLD BORLEO APONISTO, who was born on 20 November 1969 at Pontevedra, Capiz and whose parents are RUDOLFO JULIANO APONISTO and VIOLETA BALLE BORLEO.
Any person adversely affected by said petition may file his/her written opposition with this Office not later than fourteen (14) days from the date of this notice.
