In compliance with Section 5 of R.A. 9048, a notice is hereby served to the public that FLORIZA GALLETO ORSOS has filed with this office a petition for Change of First Name from “FRANCIS” to “FRANCIS JOHN” in the Certificate of Live Birth of FRANCIS JOHN GALLETO who was born on JUNE 26, 2019 at PRESIDENT ROXAS, CAPIZ and whose parents are UNKNOWN and FLORIZA DELAMIDE GALLETO.
Any person adversely affected by said petition may file his/her written petition with this office not later than fourteen (14) days from the date of this notice.
