In compliance with Section 5 R.A. Act 9048, a notice is hereby served
to the public that MA. LAILA E. CABUSLAY has fi led with this Offi ce a
petition for change of child’s fi rst name from “MARIA LAILA” to “MA.
LAILA” in the birth certifi cate of MARIA LAILA TEVES ESPAÑOL who
was born on MAY 21, 1971 at DAO, CAPIZ, PHILIPPINES and whose
parents are PABLO ESPAÑOL and LELIA TEVES.
Any person adversely aff ected by said petition may fi le her written
opposition with this offi ce not later than fourteen (14) days from the date
of this notice.
