NOTICE FOR PUBLICATION
In compliance with Section 5 of R.A. Act No. 9048, a notice is hereby
served to the public that JONATHAN D. DISTOR has fi led with this Offi ce
a petition for Change of First Name from “WILLY” to “JONATHAN”
in the birth certifi cate of WILLY DESLATE DISTOR who was born on
January 24, 1981 at Maayon, Capiz and whose parents are Rodolfo Distor
and Lydie Deslate.
Any person adversely aff ected by said petition may fi le his/her
written opposition with this Offi ce not later than fourteen (14) days from
this notice.
