NOTICE FOR PUBLICATION
In compliance with section 5 R.A. Act No. 9048, a notice is hereby
served to the public that JOCELYN B. DELA ROSA has filed with this
office a petition for change of first name from “MA. JOCELYN” to
“JOCELYN” in the birth certificate of MA. JOCELYN PELIAS BALGOS
who was born on December 8, 1966 at Panay, Capiz and whose parents are
Jose Borda Balgos and Pelia M. Pelias.
Any person adversely affected by said petition may file his written
opposition with this Office not later than (14) from the date of this notice.
