In compliance with Section 5 R.A. No. 9048, a notice is hereby
served to the public that LIZA DASO PACAÑA has fi led with this Office, a
petition for Change of First Name from “CESA” to “LIZA” and Correction
of Clerical Error of Child’s Middle Name from ORTIS to ORTIZ; Father’s
First Name from ESPEDITO to EXPEDITO in the Birth Certificate of
CESA ORTIS DASO who was born on July 23, 1971 at Panitan, Capiz and
whose parents are Espedito Daso and Rita Ortiz.
Any person adversely affected by said petition may file his/her
written opposition with this Office not later than fourteen (14) days from
the date of this notice.
