In compliance with Section 5 R.A. No. 9048, Notice is hereby served to the public that ELLEN L. MOQUERIO, has fi led with this Offi ce a petition for change of fi rst name from “ELISEO” to “ROSENDO” in the birth certifi cate of Rosendo T. Lemos who was born on June 20, 1963 at Tapaz, Capiz and whose parents are Eduardo G. Lemos and Elsa Turko. Any person adversely aff ected by said petition may fi le his/her written opposition with this Offi ce not later than fourteen (14) days after the last publication.
