In compliance with publication requirement and pursuant to OCRG
Memorandum Circular No. 2013, Guideline in the Implementation of the
Administrative Order No. 1, Series of 2012 (IRR on R.A.10172). Notice is
hereby served to the public JOAN DELA CRUZ CONDEZ has filed with
this Office a petition for Correction of Entry in Sex from “FEMALE’ to
‘MALE’ and Correction on Date of Birth from “31 2 JANUARY 1981”
to “31 JANUARY 1981”in the Birth Certificate of JOAN DELA CRUZ
CONDEZ who was born on 31 2 JANUARY 1981 at Pontevedra Capiz
and whose parents are RICARDO ANDRADE CONDEZ and CLORIA
SALAZAR DELA CRUZ.
Any person adversely affected by said petition may fi le his/her
written opposition with this office not later than fourteen (14) days from
the date of this notice.
