NOTICE TO THE PUBLIC
In compliance with Section 5 of R.A. Act No. 9048, a notice is
hereby served to the public that WILFREDO C. APOLINARIO has
fi led with this Office a petition for Change of First Name from “JOSE
WELFRED” to “WILFREDO” in the birth certificate of JOSE WELFRED
C. APOLINARIO who was born on APRIL 14, 1958 at Maayon, Capiz and
whose parents are Weldie D. Apolinario and Fredie D. Contreras.
Any person adversely affected by said petition may file his/her
written opposition with this Office not later than fourteen (14) days from
this notice.
