In connection with the publication requirement and pursuant to OCRG
Memorandum Circular No. 2013-1. Guidelines in the Implementation of
the Administrative Order No. 1 Series of 2012 (IRR on R.A. 9048), Notice
hereby served to the public that MARIO ABNER FADERUGAO, has
filed with this offi ce, a petition for CHANGE OF FIRST NAME FROM
“MARIA NILDA” TO “NILDA” in the certifi cate of live birth of his sister
MARIA NILDA FADERUGAO at POBLACION, DUMALAG, CAPIZ
and whose parents are MARTIN FADERUGAO and HILDEGARDE
FRIO.
Any person adversely affected by said petition may file his/her
written opposition with this office not later than 14 days.
