In compliance with Section 5 of R.A. No. 9048 notice is hereby served
to the public that LITO FUENTES HONRADO has filed with this office
a petition for change of first name from “JOSEPHINE” to “ELISA” and
Correction of Child’s First Name from “LANH YISHMAEL” to “LANH
NAZIH YISHMAEL” in the Birth Certifi cate of LANH YISHMAEL
MASAGCA HONRADO born on June 2, 2011 at Roxas Memorial
Provincial Hospital, Roxas City, Capiz and whose parents are Lito Fuentes
Honrado and Nelen Albaladejo Masagca.
Any person adversely affected by said petition may file his/her
written opposition with this office not later than (14) days from the date
of this notice.
