In connection with the publication requirement and pursuant to OCRG Memorandum Circular No. 2013-1. Guidelines in the Implementation of the Administrative Order No. 1 Series of 2012 (IRR on R.A. 9048).
Notice hereby served to the public that MELINDA F. MALPARTA, has filed with this office, a petition for the Correction of Child’s and Father’s Last Names from “FUSIÑGAN” to “FUSINGAN” and the Child’s Date of
Birth from “Jan 13, 69” to “January 15, 1969” in the certifi cate of live birth of MELINDA CONDINO FUSIÑGAN at Poblacion, Dumalag, Capiz and whose parents are MR. JOSE F. FUSINGAN and MRS. ESTRELLA CONDINO.
Any person adversely affected by said petition may file his/her
written opposition with this office not later than 14 days.
