In compliance with section 5 R.A. No. 9048, a notice is hereby served to the public that NIDA F. FULGAR, has filed with this office, a petition for the change of first name from “MA. LIZA” to “MARIA LIZA” in the
certificate of live birth of his father MA. LIZA FULGAR at Poblacion, Dumalag, Capiz and whose parents are LEONARDO FULGAR and JULITA FRESCO FULGAR. Any person adversely affected by said petition may file his written opposition with this Office not later than (14) days.
