O R D E R
In a verified amended petition, petitioner prays that after due notice and hearing, an Order be issued changing the name of the petitioner from PATRICIA SAN FELIX to HANNAH PATRICIA PIMENTEL SAN
FELIX and to cancel petitioner’s Certifi cate of Live Birth entered as Registry No. 98-1463 in the Local Civil Registrar of Tapaz, Capiz for all intents and purposes of the law.
Finding the instant petition suffi cient in form and substance, set the initial hearing hereof on August 8, 2023 at 8:30 in the morning at the session hall of this Court. All persons interested in the aforementioned
petition should appear and fi le their opposition to the petition. Let Copy of the order be published at the expense of the petitioner before the date of hearing in a newspaper of general circulation in the
Province of Capiz and City of Roxas, once a week for three (3) consecutive weeks.
Likewise, let copy of the order be sent to the Solicitor General, Makati City, the Provincial Prosecutor of Capiz, the Civil Registrar General, Philippine Statistics Authority, counsel and petitioner.
